EMTs, paramedics, and RNs will be completing the same application. Be sure to complete all pages of the application and sign the bottom of the last page.

Copies of all certifications relating to your level of licensure are required. For example, paramedics and RNs must include copies of state certifications, ACLS, CPR, and National Registry certification.

Photocopies of your social security card and drivers license are also necessary.

All applications will be reviewed and the applicant will be notified by mail of eligibility for future placement on our personnel pool list.
