Employment Application
Please print out the appropriate application and complete all areas of the application. EMT's and Paramedics & RNs will be completing the same applications. Be sure to complete both sides of the application and sign the bottom of the second page. Copies of all certifications relating to your level of licensure are required. For example Paramedics & RNs must include copies of state certifications, ACLS, CPR, and National Registry certification. Photocopies of your social security card and drivers license are also necessary.
All applications will be reviewed and the applicant will be notified by mail of eligibility for future placement on our personnel pool list.
